Therapy at Home is seeking a compassionate and motivated Physical Therapist to join our growing team serving older adults throughout the Milwaukee area.
We provide outpatient therapy services in assisted living communities, memory care facilities, independent living communities, and patients' homes. Our mission is to help individuals improve mobility, maintain independence, and enhance their quality of life.

Responsibilities
Perform comprehensive physical therapy evaluations
Develop individualized treatment plans based on patient goals and clinical findings
Provide skilled therapeutic interventions to improve strength, balance, mobility, endurance, and safety
Complete timely documentation using our electronic medical record system
Communicate effectively with physicians, caregivers, facility staff, and family members
Participate in interdisciplinary care planning when appropriate
Educate patients and caregivers on home exercise programs and fall prevention strategies
Maintain compliance with Medicare, Medicaid, and company documentation standards
Patient Population:
Geriatrics
Orthopedic conditions
Neurological diagnoses
Balance and fall-risk patients
Post-surgical rehabilitation
General deconditioning
Qualifications
Current Wisconsin Physical Therapist license
Valid Driver's License
Excellent communication and clinical reasoning skills
Ability to work independently while collaborating with the healthcare team
Experience in geriatrics, home health, outpatient therapy, assisted living, or skilled nursing is preferred
