We are searching for an energetic Physical Therapist Assistant to join our team full time to support Home Health patients in Columbus and the surrounding metropolitan area. The physical therapist assistant assists in the implementation of the rehabilitative care plan under the direction and supervision of a qualified physical therapist.
Responsibilities:
Under the supervision of the physical therapist, assists in evaluation and development of the rehabilitative care plan and in periodic re-evaluation as indicated.
As directed by the physical therapist, performs routine strengthening and therapeutic exercises.
Assists in teaching exercise programs to the client and family.
Consults with the physical therapist regarding the outcome of home visits and reports physical findings in written form.
Assists clients in use of self-help devices.
Attends care conferences and Agency meetings as necessary.
Documents daily progress notes on clients and submits them weekly.
Communicates effectively to all those providing client care.
Organizational Relationship:
Reports to the Director of Rehabilitation and Therapy.
Compensation: $40.00 per visit/point (minimum 35 visits a week) plus incentive opportunities.
